Tasks
and Responsibilities:
Under
the supervision of the Integrated Supports Coordinator, the implementer
facilitates activities which contribute to the achievement of the goals
in the consumer’s Plan of Care, including the following duties:
1.
Implement integrated supports activities as stated in the consumer’s
Plan of Care, including self-help and independent living skills.
2.
Prepare materials as needed for lessons.
3.
Implement intervention programs.
4.
Perform clerical and record keeping duties daily, to be turned in each
pay period.
5.
Assume responsibility for transporting individual as designated in the
Plan of Care, including maintaining insurance on personal vehicle used
for transportation.
6.
Listen to consumer and actively involve the consumer in the Plan of
Care.
7.
Maintain individual and family confidentiality.
8.
Relieve family for emergencies and other business or obligations and
notify Integrated Supports Coordinator.
9.
Keep
Integrated Supports Coordinator and family informed of any difficulties
connected with lack of understanding, health problems, or unsatisfactory
behavior.
